Please download and complete the appropriate AAO Health History Form before your first visit and bring the completed form with you. This helps us make your first appointment as efficient as possible.
- AAO Child Health History Form (PDF) — for patients age 18 and under.
- AAO Adult Health History Form (PDF) — for patients 19 and older.
Bring these to your first visit
- Completed AAO registration form (above).
- A photo ID.
- Your dental or medical insurance card, if you'd like us to verify benefits.
- Any prior x-rays, imaging, or orthodontic records from another provider.
